Collecting Your Info

Opportunity Home collects your personal information to administer housing assistance programs. Opportunity Home would like to assist you and your household by sharing your contact and personal information with community organizations that can provide you and your family with support services such as utility assistance, education, health, food access and more. Please read this notice carefully to understand what we do.

What we Collect

The types of personal information we collect and share depends on the purpose for collecting and sharing your information. Information can be identified as Personally Identifiable Information and/or Sensitive Personally Identifiable Information. Personally identifiable information is information that would not generally be considered harmful or an invasion of privacy if shared. Sensitive Personally Identifiable Information is information that when lost, compromised or shared without authorization could substantially harm an individual.

Personally Identifiable Information (PII)

  • Resident Name(s)
  • Address Information (local, permanent, and email)
  • Telephone number (local and permanent)
  • Date and place of birth
  • Housing Program, and Dates of housing assistance

Sensitive Personally Identifiable Information

  • Social Security Number
  • Driver’s License Number
  • Medical record including disability status
  • Financial account numbers such as credit or debit card numbers
  • Case Management Data such as case manager notes and social services referred and received
  • Employment information such as income and source of income

When Your Info is Collected

Opportunity Home collects information from you during the application process, the admissions process, re-examinations, and as part of other non-housing administered programs that you and your family join such as, but not limited to, the Family Self-Sufficiency Program, Jobs Plus Program, Resident Opportunities and Self-Sufficiency Program and the Education Investment Foundation’s REACH Award and Scholarships Program.

Opportunity Home may share your information at any point in time after you are enrolled in a program whether you have received services or not and and after you have stopped receiving services under a program.

Who is covered

This policy covers any active resident or previous resident of public housing or assisted housing service programs.


Who Sees Your Info

Opportunity Home may provide your personal information to our trusted business partners or persons who provide critical services under Opportunity Home programs. They receive your information based on our instructions and in compliance with our Privacy Policy and any other appropriate confidentiality and security measures which are specified under a data sharing agreement. Our partners may provide services in areas that include, but are not limited to utility assistance, education, health, food access, and many more.
