Filing a Complaint
An individual who believes they may have been discriminated against on the basis of race, color, sex, disability or national origin may file a complaint with Opportunity Home within 180 days of the alleged discrimination.

Complaints of discrimination must be filed within 180 days of the alleged incident.

Complaint Procedures
Filing a Complaint
Any individual who believes they have been discriminated against on the basis of race, color, sex, disability or national origin are encouraged to file a complaint immediately with Opportunity Home. The organization will investigate a complaint within 180 calendar days of the alleged discrimination.

Contents of Complaint
The complaint must include sufficient details of the allegation to enable the 504 coordinator to understand the nature of the situation, when it occurred and the basis of the alleged discrimination complaint. The complaint must include all contact information for the complainant, including phone number, email and address, as well as contact information of all parties and witnesses with relevant knowledge of the alleged discrimination. The complaint must be signed and dated by the complainant or their representative.
Review of Complaint
Once the complaint form is received by the 504 coordinator, it will be reviewed to determine whether the complaint constitutes a Title VI complaint. The complainant will receive an acknowledgement letter within 10 business days informing them of the 504 coordinator’s decision to investigate the complaint and, if needed, request more information for the investigation to proceed. If more information is needed, the 504 coordinator will notify the complainant, who will then be given 30 business days to provide the requested information. Failure of the complainant to provide the requested information by a certain date may result in the administrative closure of the complaint or a delay in resolution.
If the 504 coordinator determines a complaint does not constitute a Title VI complaint, the complaint will be forwarded to the appropriate department for resolution.
Investigation of Complaint
Opportunity Home has up to 60 calendar days to investigate the complaint; however, that timeframe may be extended at the sole discretion of the 504 coordinator. The complainant has 30 business days from the date of the acknowledgement letter to send requested information to the investigator of the case to avoid administrative dismissal of the case.
The 504 coordinator will take the following steps, at a minimum, to investigate the alleged discriminatory act:
- Contact and cooperate with Human Resources, General Counsel and the department the complaint originated from.
- Contact the department supervisor where the alleged discrimination occurred, determine if an investigation has been initiated and review the results of the investigation, if it was conducted.
- Identify and interview all persons with relevant knowledge, including the complainant, witnesses and supervisors.
- Identify and review all relevant documents, practices, policies and procedures to determine appropriate resolution.
Subsequent and/or Amended Complaints
Any subsequent or amended complaints should be filed using the Title VI Complaint Form. Subsequent or amended complaints can be submitted by email or another written format. The 504 coordinator will review each subsequent or amended complaint to determine whether the complaint should stand on its own or be incorporated into the original complaint. Subsequent or amended complaints could delay the timing of resolution.
Completion of Investigation/Disposition
Upon completion of the investigation, the 504 coordinator will prepare a final investigative report for the president and CEO with the investigative findings. The process and final investigation will be completed within 60 calendar days.

Complaint Appeal Process
If the complainant is not satisfied with the disposition or resolution of the complaint, and would like to appeal the decision, they may appeal directly by mail:

Appeals must be submitted in writing within 30 business days of the date of disposition/resolution of the complaint. Complaint must include the name, address and telephone number of the complainant, and must state the basis for why the complainant believes the disposition or resolution was erroneous. Opportunity Home’s President and CEO or authorized representative will set a mutually agreed upon time and place for a review and consideration of the appeal. The president/CEO or authorized representative will issue a written determination of the appeal within 15 business days of the date of the review.
